It is easy to make an appointment for your initial examination and dental treatments. Call (215) 826-3282 to speak with a staff member and you can download the New Patient Form by clicking below.

Scheduling your first appointment

Your initial dentist appointment with Dr. Clark will last about one hour. Dr. Clark will evaluate the condition of your teeth and gums. X-rays will be taken if necessary. Based on this information, Dr. Clark will develop a treatment plan and explain the options available.

Important: All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern before any procedure (i.e. high blood pressure, artificial heart valves and joints, etc.) or if you are on any medications (i.e. heart medications, aspirin, anticoagulant therapy, etc.).

X-Ray: If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office.

Insurance: If you have dental insurance, please bring you insurance card to your first appointment. We participate in all major insurance plans. If you have a question, please call our office.

Payment for Services: For your convenience, payment plans are available with no interest up to 12 months through Care Credit! We also accept Visa, Mastercard, Discover and American Express. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions about your account, please contact us at (215) 826-3282. Many times, a simple phone call will clear-up any misunderstandings.

Appointments: We will schedule your appointment as promptly as possible. If you have an emergency situation, every attempt will be made to see you that day.

Cancellations: If you are ever unable to make an appointment you have scheduled with us, please notify us at least 24 hours in advance. We would be glad to reschedule the appointment at a more convenient time.
